None of your problems get better with age, so I'm particularly sorry to hear that you have all this at such a young age.
Let me start with the problems that a mattress doesn't help. For me, my worst pain is in my neck and it's not my wife. It's the arthritis. I have tried everything. A pillow is helpful in this, but they wear out so quickly, especially if you're a hot head like me and perspire at night. The moisture will beak down most materials. Latex pillows last the longest. My wife, who also has neck pain, loves her latex pillow. I find it too thick to keep my head properly aligned with my spine...which is an important factor in minimizing that pain. Three "Extra Strength Excedrin" works in about 20 minutes. But for some, it causes stomach upset. As recent residents of California, we found something that works fast and provides pain relief that lasts a long time. If you have access to medical marijuana, they make a salve that you rub into the affected area. The pain is almost immediately and there are no side effects. That is to say, it doesn't make you high. It just works and works well.
Sleep Apnea is another issue we have in common. I too, had a cpap machine. It was the most annoying thing, trying to sleep with that mask strapped to my face. Is occurred to me that the only person really suffering from it, was my wife. So, I got her ear plugs and got rid of the machine.
You might try sleeping on a slight incline. That will also help.
Neither of you have a weight concern, so your choices for a mattress are greater.
However, the key to comfort that won't quit on you in a short couple of years, is latex. By adding a 2" soft latex topper to a reasonably firm, well made, basic mattress, you can have all the comfort and support you can want or need...and have it provide them long term.
If going with a Simmons model, plush/firm is definitely the way to go. You can't go wrong with a soft latex topper on any Simmons plush/firm model...a comfort choice that is available at any level in their line.
If money isn't a great concern, you might get the one we have at home...the Beautyrest Black Ansleigh in plush/firm. I find it to be perfectly comfortable just the way it comes. My wife, on the other hand, is more shapely and has a sore hip. With the 2" soft latex topper is feels like a million bucks....just an incredibly comfortable feel without sacrificing the firm underlying support.

There are actually undoubtedly a whole lot of details like that to take into consideration. That's a terrific point to bring up.
